What to Expect at a Tooth Extraction Appointment
- Anaesthesia: Local anaesthetic is given before any extraction. You will feel pressure and movement — not pain. If you feel pain at any point, we stop and administer more anaesthetic.
- The extraction: For a simple extraction, the tooth is rocked gently with forceps to loosen it from the periodontal ligament. For surgical extractions, a small incision is made and the tooth may be sectioned for easier removal.
- Socket care: Gauze pressure, and where appropriate, a dissolvable suture. If you are at risk for bone loss (large extraction site, preparing for an implant), we discuss socket preservation graft at this point.
- After-care instructions: No rinsing or spitting for 24 hours, soft diet, no straws (suction breaks the clot — dry socket risk), avoid smoking.

Dry socket (alveolar osteitis) occurs when the blood clot in the extraction socket is dislodged before healing is complete — usually within 3–5 days of extraction. It causes an intensely painful exposed bone socket. Highest risk: lower molar extractions, smokers, women on oral contraceptives (hormones affect clot stability), patients who use straws. Prevention: follow after-care instructions strictly. Treatment: a medicated dressing placed in the socket (immediate pain relief).
