TMJ Symptoms — What TMD Actually Feels Like
- Jaw pain or soreness — especially worse in the morning (if you grind at night) or in the evening after a day of clenching
- Clicking, popping, or grinding sounds when you open or close your mouth — most commonly when opening wide
- Limited opening — difficulty opening your mouth fully, or your jaw deviating to one side when you open
- Headaches — particularly at the temples, behind the eyes, or at the base of the skull; often confused with tension headaches
- Ear pain or fullness — the jaw joint sits directly in front of the ear canal; referred pain from the TMJ is easily confused with ear infections
- Facial muscle soreness — especially in the masseter (the chewing muscle at the angle of the jaw) and the temples
- Neck and shoulder pain — can radiate from chronic jaw muscle tension

We follow a conservative-first protocol because the research strongly supports it — 70–80% of TMD patients improve significantly with non-surgical treatment. We do not recommend aggressive irreversible treatments (bite adjustment, orthodontics for TMD) without evidence of sustained failure of conservative methods.
- Occlusal splint (night guard): Custom-fitted hard acrylic guard worn during sleep. Prevents grinding from reaching the teeth and joint; repositions the jaw in a more comfortable position. First-line treatment for most cases.
- Physiotherapy referral: Manual therapy of the TMJ and associated neck/shoulder muscles significantly improves outcomes in most cases. We can refer to Hamilton physios who specialize in craniofacial work.
- Anti-inflammatory protocol: Short-term NSAIDs (ibuprofen) during acute flares
- Botox for masseteric hypertrophy: For patients with significant masseter enlargement from chronic clenching, Botox injections into the masseter muscle reduce the bite force substantially. Available at Red Rose.
